Supported Living

We provide bespoke supported living services that encourage and motivate people to do as much as they can for themselves enabling them to gain practical and cognitive skills.

Our supported living services enable individuals with different challenges to live the lives they want and have the opportunities they deserve. Our services enable one to live a more independent life, have more choice and control.

Our Key Support Workers take time to listen to each individual to understand what they want to achieve in life. We work closely with them, their family, circle of support, and other healthcare professionals to craft a support plan and also to achieve goals stated in the support plan. We use an outcome-focused approach that provides support designed around each person’s needs.

Learning Disabilities

Our team are experts in supporting individuals with learning disabilities using Positive Behaviour Support. Our team monitors the behavioural, emotional, and psychological needs of those we support.  A holistic approach that considers an individual life history, personality, physical health, and emotional needs is used.

A care plan will be drafted with the help of the individual, their family, and also other healthcare professionals such as psychiatrists, psychologists, social workers, etc. All care plans promote safety, consistency, and reliability.

Autism

Aqua Robson Care has adopted a tailored approach involving individuals with Autism, their family, and other healthcare professionals to provide support and help them develop functional abilities and skills that improve the quality of their lives. This allows the crafting of a bespoke care plan that is regularly reviewed to reflect individual development and progress. These plans detail how each individual should be supported, their preferences, how their environment should be made suitable for them, and how to ensure that all their needs be it physiological, physical, psychological, or emotional are met.

We take note of recommendations from Sensory Assessments, Occupational Therapists, Speech and Language Therapy, and Psychologists. We encourage self-confidence and independence.

Mental Health Needs

We support individuals to make informed decisions about important things that affect our daily lives. Our specialist support helps to identify personal needs, challenges, and aspirations and helps to build long-term self-management strategies.

We work with social services and community home teams to craft bespoke care plans that are monitored regularly. We employ tools such as Recovery Star to measure achievements. Individuals set their own goals and this helps to reinforce motivation, enhance skills and confidence and also assist in the identification of objectives and ways to achieve them.

Our Support Plans

We ensure that everyone receives personalised care and support plan which they have control over and can develop with our staff and their family.  Our support plans are formed using the principles of the Outcomes Star Model which is recognised by the Department of Health. The Outcomes Star Model focuses on;

  • Managing mental health
  • Enhancing living skills
  • Building and maintaining social connections
  • Maximising independence

The support plan will include:

  • Managing finances and tenancy.
  • Socialising with friends and family.
  • Support to attend appointments.
  • Enjoying hobbies or interests.
  • Attending college, work placements, and training opportunities.
  • Advice and advocacy.
  • Developing domestic and life skills such as cooking and cleaning.
  • Getting out and about in the community.
  • Keeping safe and healthy.
  • Emotional support.
  • Help to access other services.

We aim to give support that allows individuals within our care to gain the confidence and life skills needed to realise their personal potential, achieve positive outcomes, and promote independent living.
